Partnering with HHCA

vision.jpg

Housing for Health partners with providers to expand access to coordinated housing and health services across California.

Below is an overview of the process for becoming an HHCA partner, including key steps and what to expect along the way. Our goal is to set every provider up for success through a thoughtful onboarding process grounded in shared values and clear expectations.

Step 1

Initial Discussion

The process begins with a 30–60 minute introductory meeting to explore the engagement and assess potential alignment.

Step 2

Review of Terms

HHCA presents a program term sheet outlining expectations, structure, and requirements.

Step 3

Readiness Assessment

If both parties agree to move forward, a readiness information session and assessments are scheduled within 5–7 business days, subject to availability. Providers must achieve a passing score of 80% or higher on each assessment, with the number of assessments varying based on the services a provider intends to offer.

Step 4

MOU Execution and Training Initiation

Providers who pass the readiness assessment proceed to signing the Memorandum of Understanding, which requires a two-year commitment. Both parties then work together to establish a training schedule. Training is mandatory and includes six live sessions.
