At Housing for Health, we understand that navigating the path to stable housing can be challenging. That’s why we offer whole person care through a comprehensive range of services designed to support and empower individuals and families experiencing or at risk of homelessness. We provide guidance, resources, and a helping hand, ensuring that all Californians have the opportunity to build a healthier, more secure future.
Housing Navigation Services
Finding a safe place to call home is the first step towards stability.
Housing Navigation services include assisting with document readiness, resolving debt and credit concerns, applying for a housing voucher, landlord engagement, and move-in assistance. Many clients who receive Housing Navigation Services are also enrolled in Housing Sustainability and Tenancy Services once move-in is complete, allowing them to receive important support services that support their long-term housing sustainability and improvements in their quality of life.
Housing Deposits
Our Housing Deposits program provides crucial financial assistance to help with rental and move-in costs.
Clients who are enrolled in Housing Navigation Services are eligible for a one-time benefit of up to $5,000.00 to assist with securing a housing unit, including the first month of rent and a deposit. This benefit also allows for basic household items that are necessary to accommodate the members transition into housing.
Housing Sustainability & Tenancy
Maintaining stable housing is a journey, and we're here to support our clients every step of the way.
Our Housing Sustainability & Tenancy Services are offered once an individual has been housed, and provide support in financial management, community integration, and health management. We work with our clients to develop the skills and resources they need to thrive in their new homes, ensuring lasting stability and well-being.
HOUSING SERVICES
Day Habilitation
Day Habilitation services help individuals at risk of or experiencing homelessness build the skills they need for a better quality of life and stable housing.
The program covers practical topics like financial literacy and job training to equip participants with the tools for independent living. With flexible options available through one-on-one or classroom-based sessions, the program adapts to meet each person's unique needs.
Enhanced Care Management
We understand that complex health needs can present unique challenges.
Our Enhanced Care Management program employs a whole person approach to care while addressing both the clinical and non-clinical needs of complex members. These individuals still qualify for Housing for Health’s entire suite of services but also receive oversight from a lead care manager who can provide continuous, customized support for each client’s unique needs.
Short-term Post-Hospitalization
Transitioning from a hospital stay to stable housing can be a challenging process.
Our Short-term Post-Hospitalization Housing Support program provides temporary housing to support individuals transitioning from hospitalization who need a safe, stable environment to continue their recovery. Whether healing from injuries, illnesses, or behavioral health conditions, this program provides the critical support needed to prevent setbacks and promote lasting well-being.
